Label - The text that is written before/above the input field.
Field name - The name of the key in the custom database table. See Manage Keys for more information. You must create a key before you can use it as a field name in this dialog box.
Description - A short description of this form field.
Type - The form item type can be one of the following: Edit Box, Combo Box (Edit), or Combo Box (Fixed). The edit box is a simple text field where the user can enter information. The Combo Box (Edit) is a combined text field and drop down box with options the user can choose from, and finally, the Combo Box (Fixed) is a drop down list with options the user can choose from, but the user cannot enter any new information in this field.
Values - The values list is only active when choosing one of the two Combo Box types above. Add items for the drop drown box in this list.
User must fill in this field - Check this option to require that the user fills in the field before proceeding to the next page or finishing the user form.
