Adding a questionnaire

In Questionnaire Administration, the user adds a questionnaire. The user can do other tasks, such as request certification from the attester that specific documents have been read. The user may select that the questions be displayed in a random order.

The attesters must attain the certification before they can begin answering the questionnaire. The certification emphasizes the attester's awareness of certain policies and procedures.

To add a questionnaire

  1. On the Home page, click Administration.

  2. In the Questionnaire Administration page, click Add New Questionnaire.

  3. Click Browse. Navigate to the questionnaire directory.

  4. Select the file. Click Open.

  5. Click Upload to add the questionnaire to the attester's queue.
